The Incidents module allows Contractors to report workplace incidents directly, ensuring their safety concerns are documented and addressed promptly. This feature promotes transparency, compliance, and a safer working environment for all parties involved.
To report an incident, you must be assigned to at least one Site for the Client for whom you are reporting the incident.
To report an incident in Contractor web app, follow these steps:
1. Select the module picker > Incidents to open the Incident List area.
2. Next, select Report Incident in the upper right corner.
3. Select the Client and Site where you wish to create the incident report.
The Client and Site dropdown will only display the active clients and sites that are assigned to you.
4. Once the Client field is selected, the incident types configured by the client will appear below the Date Reported field. Enter the required details for the incident and select at least one incident type.
a. Select the Client where the incident occurred
b. Select the Site where the incident occurred
c. Provide a Short Description of the Incident (used as the incident label)
d. Select the Incident Shift Time when the incident occured (e.g. In the first 25% of the shift, In Overtime, etc.)
e. Select the Incident Date & Time
f. Select the Date Reported
g. Select one or more Incident Types in the section βI want to report aβ
βNote: Typically, only one Incident Type is selected per incident. However, in exceptional cases where a critical Incident involves multiple factors, you may need to select multiple Incident Types to ensure comprehensive documentation of the Incident.
The Incident Types displayed in this section are company-specific and may vary. The options shown in the image are examples only.
5. When completed, select Continue at the bottom right to save the incident report and proceed to the next step.
6. If Potentially Notifiable Incidents are configured for your selected Client, you may get the prompt below. For more information, refer to the Potentially Notifiable Incidents workflow, which works the same in the Contractor web app.
7. Select an Incident section and add detailed information in the next part of the form for the chosen incident type(s).
NOTE:
Any changes you make will be automatically saved while you are working on each section in the Incident form.
Corrective Actions and Investigations will be hidden from the form
8. Select Finish when you have completed the incident report.
Upon selecting Finish, the Site Contact for the selected Site will automatically be notified about the incident. If a notification group is triggered, the Notification Manager will be notified instead.
