16-02-2024 Incident V9 release

Incident APP

  1. Incident Form: Adding Corrective Actions

    • The CTA button can no longer be clicked multiple times, preventing potential data issues

    • Users adding a new Corrective Action by clicking + Add Corrective Action also have the option to delete the Corrective Action if it is no longer required


  2. Incident Form in edit mode

    • The notification message/information displayed when the User selects Notify or Finish button and there are still required questions not filled has been enhanced to improve the User experience (UX) for both required questions and Investigations

  3. Investigations page

    • Security measures have been updated to prevent cross-site scripting (XSS) payloads when completing and saving Investigations

  4. Creating a new or editing an existing Incident

    • Security measures have been updated to prevent cross-site scripting (XSS) payloads when completing and saving the Incident Description field and the Incident Location field

  5. Closing out Incident

    • The process has been fixed to function properly when closing out Incident that has a Corrective Action and the Corrective Action Staff creator has been deleted for any reason

  6. Reports page

    • The issue with the top right profile dropdown overlapping User Interface (UI) on the Reports page has been resolved.

  7. Incident view mode

    • An option has been included to Staff User with Incident R permission to add Corrective Actions (the CTA button was missing before)

  8. Incident Form Details view mode

    • The Sign-off reviewer and Incident Sign-off alert will be reset and will not be displayed if the Incident status changes for any reason

  9. Contributing Factor Details view

    • The Corrective Actions List and Details in the Contributing Factor Details view have been fixed & updated to align with the new System Design.

  10. Incident Initial Notification Email Alert

    • The text ‘Incident has not been classified’ is no longer displayed when no Tags are triggered

  11. Incident Module dropdown

    • Alphabetical ordering has been implemented across several dropdown fields

  12. Staff User forwarding the Incident notification

    • The validation error message for the Additional Email Address field has been resolved, specifically when the maximum number of allowed emails is reached

  13. Incident Form detail view

    • When adding activities under Corrective Actions and Investigation, the ‘confirmation browser dialogue’ to save the changes has been fixed to ensure that it is only triggered when expected

    • The file links are now operational, allowing the User to download the files

  14. Creating / Editing an Incident Chart

    • The values in the Incident Question dropdown have been contained within the designated area and no longer extend beyond it

  15. Notification List improvements

    1. CSV export - Person Responsible column data fixed

    2. Person Responsible column will show if the Staff User has been deactivated

  16. Incident Form

    • The validation issue for the Injury Table related to postcodes has been resolved

  17. Incident Email Notification

    • When using the Forward Notification feature in the Incident module and choosing the Email Notification option with a PDF attachment, the Client logo has been incorporated into the top left-hand area of the existing template in the PDF attachment

  18. Add / Edit Incident

    • Enhanced the validation for required fields in File and free text question types

  19. Incident module: Notify modal

    • The issue with the display of additional text for Notification Manager and Notification Groups in relation to the use of the Bypass Person Responsible feature has been resolved

  20. Incident Form

    • The Save button has been enabled, providing Users with the capability to save modifications made to the Hierarchy of Control field within any Corrective Actions.

    • The date of birth (DOB) field will be automatically filled in once a person is selected from the ComplyFlow system within the Injury Section.

  21. Allow Email Forwarding feature

    • The Additional Emails field will be managed by a new radio button field

    • If Yes is selected, it is necessary to enter at least one email address

    • If No is selected, the Additional Emails field will be hidden

  22. Creating a new Incident and selecting the Notifiable option

    • When the User has the Notifiable modal on screen and attempts to navigate away or refresh the browser for any reason, the process has been improved to display a confirmation browser dialogue

    • If the User chooses to navigate away, the Incident will not be created. This will prevent problems by creating restricted incidents and not notifying people

  23. Investigations List: Actions

    • The label Start Investigation has been updated to Update Incident

  24. Incident edit mode

    • The label for the Description field has been updated to File Description in the File question type

    • The label for the Person Responsible field has been updated to Notification Manager(s) and a tooltip has been added in the Notify modal

  25. Incident Form Detail

    • The information in the Submitted By and Last updated By fields has been enhanced to provide greater clarity

  26. New Email Alert: ‘Weekly Email digest of pending Incidents / Outstanding actions

    • This will trigger an email alert resembling the Dashboard reminder, ensuring that Staff Users receive a comprehensive report on a weekly basis covering all outstanding Incidents and Actions

  27. Notifications

    • An option for attaching a PDF report has been included for both submitting the initial Incident Notification and forwarding it

  28. Incidents with more than one Incident Type

    • While in edit mode, the duplicated sections will be concealed and rendered unnecessary

    • The Incident Form details view and PDF report have been updated

    • Modifying the Incident and eliminating any Incident type(s) will also erase the data from all question-answers across all sections related to these Incident types

16-02-2024 February System Release V2

  1. Incident APP

    • The CSV export for Download All has been updated to restrict access to only the Incident Admin Staff User(s) in order to view the CTA button and download

  2. Incident APP: Notifiable Incident

    • Enhancement of Notifiable Incident modal validation to improve the User experience (UX)

  3. Incident APP: Configuration: Reporting Columns

    • The Incident Type column has been removed as it was no longer necessary while the functionality remains unchanged

  4. Incident APP

    • The Sign-off alert has been updated to include the System design within the Dashboard module and Incident Reports view

  5. Live Access API: Creating Forms / Permits

    • The process has been enhanced to include the option to delete Permits if the User decides to cancel instead of submitting the Permit. This change will not affect the web application in any way

  6. Client App: New Staff List

    • The process for adding Staff has been optimised to ensure that the Name field updates correctly across the entire OCF System both before registration and after. No other changes have been made to the Add Staff Wizard flow and functionality

  7. Client App: Dashboard

    • The issue has been resolved so that Form Alerts linked to Contractors display Contractor as the Entity, whereas Form Alerts not associated with any Company display Staff as the Entity

  8. Inspection App: Templates

    • When the Checklist Items and Categories are uploaded using the CSV drop zone area, the validation error message text has been updated

  9. Inspection App

    • When a new Inspection is being created or an existing one is being edited, the validation for the Date field has been enhanced to prevent Users from entering an invalid date

    • The Security access has been fixed so that only the Staff User(s) with Audit Admin rights can view and access the menu and submenus

    • The navigation within the Setting menus has been rectified to ensure that the menu remains expanded and highlighted accurately

    • The email alert for ‘New Inspection Actions assigned to you’ has been updated to fix typos and improve its contents, enhancing the overall User experience (UX)

  10. Contractor APP

    • The Add Worker Wizard Step 2 has been disabled for Worker Admins associated with at least one Enterprise Client

    • The Account Status filter under Worker List has been fixed to ensure that the list view displays the data correctly based on the applied filter(s)
